If you are caring for older parents, spouses or other loved ones, helping them to live independently in their own homes, you should know about this law which took effect on June 9, 2016.

As a caregiver, you have  a huge responsibility, and the CARE Act is intended to make life a little bit easier for you. This law requires hospitals to:

 Provide your loved one the opportunity to designate a lay family caregiver
 Inform you when your loved one is to be discharged to another facility or back home

 Provide instruction or training on the aftercare tasks you will need to perform at home

This provision does not replace Advance Care Planning and designated Power of Attorney for Healthcare Decisions, but it can provide support if these documents are not in place.

You can get more information and download a wallet card at the link provided.
